Call 4 Concern
What is Call 4 Concern©?
Call 4 Concern is an inpatient safety service at the Countess of Chester Hospital.
It enables patients and their families to call for help and support if they feel that the health care team has not recognised a deterioration in their own or loved one’s condition.
Call 4 Concern is run by the Critical Care Outreach Team and is available 24 hours a day to help support patients, families and ward teams.
When to make a Call 4 Concern:
Contact Call 4 Concern directly if:
• You feel that there has been a noticeable deterioration in your own or loved one’s condition and you feel that the healthcare team is not recognising your concern.
• You have ongoing concerns after you have spoken to the ward nurse or doctor.
When not to make a Call 4 Concern:
Call 4 Concern is a patient safety service. To report problems regarding your hospital bed, room, food, parking or any other general issues, please speak to your nurse, ward manager or PALS (Patient Advice and Liaison Service)
Responding to your call:
When the Call 4 Concern team receive your call, they will need to know:
• Your name (if you are the patient) or the name of the patient you are concerned about.
• The name of the ward.
• A brief description of your concern.
If the team are busy dealing with a sick patient and unable to take your call, they will politely request that you call back or will take your contact details and return the call at the first opportunity. We thank you for your patience and understanding.
What happens next:
After prioritising the urgency of the problem, a member of the Critical Care Outreach Team will visit the ward, assess the patient and liaise with the responsible medical team to decide on the best course of action.
If the patient is in agreement someone from the team will then contact you to discuss whether your concerns have been addressed.
Please note, we realise that a concern is not a complaint. Do not feel concerned that using this service will negatively impact a patient’s care in any way.
