Cardiac Rehabilitation Service (Integrated Care)
The Cardiac Rehabilitation Services at the Countess of Chester Hospital provides follow on care for anyone who has had:
- Acute coronary syndrome (including ST elevation Myocardial Infarction, (STEMI) non-STEMI (NSTEMI) Myocardial infarction/Heart Attacks
- All patients undergoing coronary reperfusion /surgical revascularisation (Coronary Artery Bypass Grafting (CABG), or ‘stenting/angioplasty’ (PPCI/ PCI)
- Patients who have undergone surgery for implantation of intra-cardiac defibrillator (ICD)or cardiac resynchronisation therapy after a heart attack or surgical revascularisation (CRT-P, CRT -D)
- After a heart valve replacements or heart valve repair.
We also host a number of specialist services providing our patients with;
- Symptom management and review
- Medication optimisation (specialist nurses)
- Activity and Exercise (exercise physiologists)
- Cardiovascular disease risk factor management (blood pressure and cholesterol management);
- Long term management of a heart condition
- Exercise and fitness advice
- Emotional well being/psychological health support
- Food and heathy eating advice
- Advice on returning to your normal activity after a heart attack/procedure
- Help returning to work and gettingback to normal.
The service has been a Nationally Accredited service since June 2015 and was one of the first in the country to receive accreditation. It submits data to and as part of the National Audit of Cardiac Rehabilitation Outcomes Report.
The Cardiology team organise clinics on site at The Countess of Chester Hospital, a clinic twice weekly at Ellesmere Port Cottage Hospital and exercise and wellbeing sessions which are run locally at the ‘1st Upton Scout Hut’.
If you feel you need further advice and support, the team also provide diabetes specialist nursing and talking therapies (cognitive behavioural therapy (CBT), and counselling). All our patients have access to our range of services relevant to your needs and the service is very much tailored to meet your clinical needs and personal goals. The programme starts with an initial phone call, then a face to face clinical appointment (ECG and clinical assessment). If required, you may then access exercise /mobility /lifestyle/ and wellbeing sessions as needed.
Ongoing cardiac rehab appointments can be face to face, with a manual/book, via telephone or a mixture of whatever you feel you need.

Personalised information can be given to all patients and this can be tailored the individual. It can be sent via post in a booklet or via email. Patients can also access Cardiac Rehab ‘Online’ via a validated Heart Manual or a resource called REACH HF.
